Everything You Need to Feel Prepared for Your Visit

Financial Policy
We believe that clear communication helps create a smoother experience for our patients. To help you understand our billing practices, payment expectations, and insurance guidelines, we’ve provided a copy of our Financial Policy.
Please take a few moments to review the document before your appointment. If you have any questions, our team will be happy to assist you.
FOR YOUR APPOINTMENT
If you have x-rays or other studies or records which you believe would be helpful in understanding your condition, please bring them with you.
Also bring the name and address of your referring physician so we can keep him or her advised of your condition.
You will receive pre-registration forms by mail in advance of your appointment. Please fill these out before your appointment. It will speed your visit, and ensure that we have all the information necessary to give you the best care possible.
The information you give to us is confidential and will not be released to anyone without your written authorization.
On your arrival, our receptionist will register you at the front desk.
We respect your time and will be as efficient as possible in completing your thorough exam. Our office has x-ray facilities if images are needed for a complete evaluation.
Please be certain to bring to your appointment all the items in the following check list that apply to you:
- Information sent by us completed
- Insurance cards
- Drivers license
- Pertinent information from your referring physician
- Recent x-rays – including bone scans, CT scans, MRI scans, arthrograms, or EMGs
- Written referral voucher from your primary physician (if this is your insurance requirement)
- Workers compensation, No Fault (auto accident) Second Opinion Evaluation, and legal problems MUST have a written authorization from the third party responsible for paying your bill, if you are not prepared to pay personally.
- A family member or friend that you wish to be informed about your condition.
The length of your visit depends on the complexity of your case as well as the complexity of others that are being seen that day. An initial complete evaluation may take from one to three hours.
Appointments
Please call the office to make an appointment in advance.
If you are unable to keep your appointment, please call us as far in advance as possible so we may use that time to see another patient in need of care.
Any appointment rescheduled or cancelled within 24 hours of the scheduled time is subject to a $50 charge.
We make a sincere effort to adhere to our appointment schedule and appreciate your patience if we are late due to emergencies or hospital surgery.
Phone Calls
When you call and request to speak to the doctor or physician assistant, we will do our best to return your call the same day.
Emergency Situations
Should you have an emergency during business hours, you may call (517) 267-0200. Your message will be relayed to your doctor or physician on call. If after business hours and/or you cannot wait for a return call, please dial 911 or proceed to the nearest emergency room.
Billing
We will submit your insurance claim. If you have co-pays and/or deductibles, please pay at the time of your office visit. If you have no insurance, payment will be expected at the time of your office visit. Payment or a deposit may be requested in advance of elective surgery if no insurance coverage is available for the service. We accept cash, checks, and credit cards for your convenience.
Fees
You will find that our office and surgical fees are similar to those charged by other orthopedic and joint reconstruction specialists located in this area of the country.
Prescriptions & Renewals
For your convenience, prescription renewals are available via our patient portal.
Renewal requests are reviewed during business hours.

Our administrative staff will explain your coverage, copays, and any out-of-pocket costs in simple, clear terms.

Call us if you have questions regarding our participation with your plan. It is your responsibility to advise us of any requirements of your policy for prior authorization, pre-certification, or a second opinion. When a plan requires you to be referred by your primary physician, it is your responsibility to bring this written referral or authorization with you to your visit.
Please recognize that as the patient, you are the person ultimately responsible for the payment of your bill, regardless of your insurance status. As a service to our patients, we are happy to submit insurance claims.
In cases of liability or workers compensation, we do not consider the existence of ongoing legal action a valid reason for delaying payment of your account. The patient or their guardian is responsible for prompt payment of bills regardless of litigation that may be simultaneously occurring. When payment is received for services, we promptly fulfill our responsibility to provide your attorney with information about your treatment.
